پنوموکوک قبل واکسیناسیون
مهم ترین عامل عفونت های مهاجم باکتریال محسوب می شد
همچنین مهم ترین عامل مرک ومیردر تمام سنین محسوب می شد
بعد از یک قرن تحقیق در مورد پنوموکوک هنوز بسیاری از مسائل در
ابهام وحل نشده است
از دیگر مسائل مقاومت داروئی در مورد پنوموکوک است
Most common bacterial cause of:
Bacteremia
Meningitis
Pneumonia
Otitis media
Mastoiditis
Sinusitis
Conjunctivitis
Most common bacterial cause of :
Bone & joint infections
Soft tissue infections
Peritonitis
Endocarditis
Pericarditis
Infection in immunocompromised hosts
Meningitis
S. pneumonia the 2th most frequent after( N.Meningitidis
)cause of B.M. in countries in which routine vaccinating against
Hib has been introduced
S.pneum. Most commonly invades the meningitis via the
bloodstream
In rare cases it can invade the meninges directly

After penetrating trauma or
Fracture of the base of the skull
CSF leak ,
HIV infection
Sickle cell anemia
Asplenia
Are predisposed to the development of Pneum. Meningitis
Signs & Symptoms
Rapid onset(
< 24 hr of illness )
Occurs in less than 20 % of children with pneumococcal
meningitis
Signs :
Fever , Nuchal rigidity , irritability or Lethargy and poor
feeding , the anterior fontanelle often bulging
Signs & Symptoms
Seizures about 25% of patients
Decreased consciusness & septic shock occur in 11% -16 %
The course pneumococcal meningitis can last for more than 10
days
The course can be attributed to complications , subdural
effusion or empyema
Signs & Symptoms
The mortality rate varies from 6.3% -20 %
Hearing loss 30 %

CSF ( sugar ) = 25 mg/dl
CSF ( WBC ) = 62000 ,90 % poly,
10% Lymph
RBC 300
Protein ,52 mg/ dl
Treatment start with : Ceftriaxon 50 mg/k /dose
Every 12hr & Vancomycin 15 mg /k/doseEvery 6 hr+
Dexamethazon
Case Report very Rare
CSF smear Gram Positive
CSF culture Pneumoccocus
Blood Culture Negative
U/C negative
Urea 20 mg/dl
Creatinine 0/5 mg/dl
واکسن کنژوگه و غیرکنژوگه پنوموکوک
واکسن غیر کنژوگه که بعد از 2 سالگی باید تلقیح شود
اما واکسن کنژوگه زیر 2 سال تلقیح می شود

13 گونه پنوموکوک را پوشش می دهد ودر 2-4-6-15ماهگی می توان به کار
برد
هم اکنون بیش از 100 کشور جهان از این واکسن استفاده میکنند
در 41 کشور جهان و6 کشور عربی جزء واکسیناسیون ملی وروتین محسوب می
شود
توصیه می شود
